Fractyl Health Meets REMAIN-1 Pivotal Endpoint, Shares Plunge

Hitting a pivotal endpoint is supposed to be the good day. Fractyl Health hit its, with a p-value of 0.0023, and the stock still lost about half its value. Opus Genetics had its own odd Friday: an FDA approval a day early, and shares closed lower.

Opus Genetics Wins FDA Approval for Presbyopia Eye Drops, Shares Slip

Opus Genetics (Nasdaq: IRD) said on October 16 the FDA approved its phentolamine ophthalmic solution 0.75% for presbyopia in adults, one day ahead of its October 17 PDUFA date, the FDA’s deadline for a decision. The drug already sells as Ryzumvi for reversing drug-induced pupil dilation. This is a label expansion and the company’s second U.S. approval.

Two Phase 3 trials, VEGA-2 and VEGA-3, met their primary and all key secondary endpoints with no treatment-related serious adverse events. Eye redness showed up in 21% of trial subjects. By the company’s count, presbyopia affects about 90% of U.S. adults over 45.

Opus won’t sell it. Viatris holds exclusive U.S. rights, and the approval triggers a milestone payment of undisclosed size. No presbyopia launch date or price has been announced.

The stock closed roughly 5% lower on the day, leaving a market cap near $319 million. Insiders sold a net $19.3 million of stock over the past year.

Fractyl Health’s Revita Meets Pivotal Endpoint as Shares Plunge

Fractyl Health (Nasdaq: GUTS) reported on October 16 that its REMAIN-1 pivotal study met its six-month co-primary endpoint. Patients treated once with Revita, an investigational duodenal mucosal resurfacing device, kept about 80% of their GLP-1-induced weight loss after stopping the drugs. Sham patients kept 67%. The p-value was 0.0023, and the gap between arms nearly doubled from month three to month six.

Shares traded down about 49% early and fell more than 58% at one point, per two separate reports. Market cap now sits near $262 million.

The reaction has history. Fractyl shares collapsed roughly 68% to 75% over two days in January when fuller midpoint-cohort data and site-level issues came out. Multiple securities class actions now allege the company overstated Revita’s efficacy and the integrity of that earlier data, covering purchases from January 13, 2025 to January 29, 2026. The lead-plaintiff deadline is today, October 20. None of the claims have been proven.

Revita holds Breakthrough Device Designation, which is not an approval. A De Novo filing, the FDA route for novel devices with no existing equivalent, is planned for late Q4 2026. The second co-primary endpoint, the share of patients holding at least 5% total body weight loss, comes with one-year data in Q1 2027.

XBiotech Antibody Clears Phase II Survival Bar in Colorectal Cancer

XBiotech (Nasdaq: XBIT) reported final Phase II TASKIN results on October 19 from an investigator-sponsored trial funded by France’s National Cancer Institute. Vilamakitug, an investigational antibody targeting IL-1α, was added to standard chemotherapy in heavily pretreated metastatic colorectal cancer. Forty-nine of 80 patients on it were alive at 12 months. The protocol needed 42.

Check the control arm. Forty-four of 79 placebo patients were also alive at 12 months. The trial’s Simon two-stage design set a survival threshold in advance rather than requiring a statistical win over placebo. Twelve-month time to strategy failure was 31.2% versus 20.3%, not statistically significant (p=0.17). The strongest signal, in right-sided tumors, comes from an exploratory analysis that was not pre-specified.

Market cap was about $66 million on October 8, against roughly $109.5 million in cash. Sushma Shivaswamy runs the company as interim CEO, after founder John Simard announced his retirement in December 2025. Law firms announced securities investigations in January 2025 after a rheumatoid arthritis program missed its primary endpoint; aktiego.com could not confirm their current status. XBiotech says TASKIN points toward registrational development. No timeline yet.

IRD, GUTS, XBIT: Forward-Looking FAQ

Does the presbyopia approval put money in Opus Genetics’ pocket right away?

Some. The approval triggers a Viatris milestone payment of undisclosed size, and Viatris controls U.S. sales with no presbyopia launch date or price announced yet.

Does today’s lead-plaintiff deadline in the Fractyl class actions change its De Novo timeline?

Not on the company’s stated schedule, which still targets a late Q4 2026 filing and one-year REMAIN-1 data in Q1 2027. The cost and outcome of the litigation remain unknown.

With 44 of 79 placebo patients also alive at 12 months, is vilamakitug’s survival benefit proven?

No. TASKIN cleared a threshold set in advance, the time-to-strategy-failure gap was not significant (p=0.17), and the right-sided tumor signal is exploratory, so a registrational trial would need to confirm any benefit.
